Well Tags

Overview:

Any hole made in the ground to explore for groundwater, to obtain, or to inject water into any underground formation from which groundwater may be produced must have a well tag. 

Wells must be drilled by a licensed well driller. Please see the Maryland State Board of Well Drillers for more information on Well Drillers and to obtain a current Roster of Active/Current Licensees and to learn about the Application Process.

Well Forms 

All forms are multi-page forms provided by the State with sequence numbers. Forms may be obtained by the Licensed Well Driller from the Environmental Inspection Services Program at 1001 E. Fayette St., Baltimore, MD 21202. The following are to be used as reference only. Please refer to the Permit Guide 3.14 for guidance on well construction permits.

Checks or money orders must be made payable to the  Director of Finance
