The mission of the Baltimore’s City Health Department’s Field Health Services (FHS) program is to provide Non-Emergency Medical Transportation (NEMT) to and from medically necessary medical appointments for recipients of Medical Assistance in the City of Baltimore.

The FHS/NEMT program bridges the gap for Medical Assistance recipients by providing transportation to and from medically necessary appointments for Medical Assistant recipients as a last resort resource.

Eligibility:

In order to receive transport you must have:

  • A medically necessary medical appointment
  • Full active Medical Assistance Coverage
  • Live in Baltimore City
  • Have no other means of transportation available (Medical Assistance Transportation is the last resort)—
    • no car or unable to drive,
    • no family/friends who can drive you,
    • do not live on a bus route or Mobility Transport route, and
    • not eligible to ride with Mobility Transportation Authority (MTA)

The Maryland State Department of Health requires medical certification for all recipients. The original PCF must be signed by the patient’s designated medical provider and mailed to the Baltimore City Department of Health
