Baltimore City Local Health Improvement Coalition (LHIC)
The Baltimore City Local Health Improvement Coalition (LHIC) is a state-mandated convening body of key stakeholders led by the Baltimore City Health Department for the purposes of supporting the improvement of health outcomes throughout Baltimore City. Representatives from healthcare systems, community-based organizations, faith-based institutions, businesses, foundations, and community members from across the city convene to drive improved outcomes at the population level for three public health priorities: diabetes, care coordination, and social determinants of health (SDOH).
To protect health, eliminate disparities, and enhance the wellbeing of everyone in our community through education, coordination, advocacy, and direct service delivery.
An equitable, just, and well Baltimore where everyone has the opportunity to be healthy and to thrive.
We value everyone. We practice selflessness and empathy. We are collaborative and respect others’ input. We ensure thoughtful, transparent, and intentional communication.
We serve and work with integrity. We hold each other and ourselves accountable through an equitable lens.
We protect people and empower them. We seek to change lives. We meet people where they are and do it with empathy.
We trust the evidence. We make evidence-based and community-informed decisions.
We embrace bold ideas. We are innovative risk takers with diversity of thought. We are willing to try new things and we aim to be creative in our approach, always keeping quality of care as a priority. We lead the field of public health with boldness and courage.
