Maryland Access Point (MAP)

Maryland Access Point (MAP) is the state project for the national Aging and Disability Resource Center (ADRC). It is a “no wrong door” initiative that integrates aging and disability services and provides support to allow individuals to remain at home and plan for future needs. Baltimore City MAP provides information by telephone regarding resources for adults with disabilities and older adults and serves as a “one stop” shop source of information for older adults, their families and caregivers. Callers and/or inquirers will receive information, assistance, advocacy, referral and follow up to assist with a wide range of concerns including screening for federal and state benefits, housing, legal, nutrition, and other community based services, programs and resources.   • Benefits Check Up: MAP administers a free and confidential questionnaire thats helps to determine programs and services that an individual may be eligible for on a local state and federal level. Individuals 55 and older can complete the questionnaire attached and mail it to 417 East Fayette Street, 6th floor, Baltimore, Maryland 21202.   • Options Counseling: MAP promotes long term care planning and community living through a person-centered approach.  Contact a counselor at 410-396-2273 to discuss longterm care goals and support services.
  • Resource Request: MAP provides resources and educational materials for personal needs, community members or community events - i.e: Resource directories, gold cards, housing lists, taxi card application, water discount application,  energy assistance application.
