The Field Health Services Program is Moving

On Monday, August 5, 2019 and Tuesday, August 6, 2019 the program will be relocating to 1200 E. Fayette Street.  During these two days all Clients & Community Partners will be directed to the program’s transportation vendor, Hart to Hart Transportation at 443-573-2037, to directly schedule transportation. This information will be shared on the Health Department's Facebook, Twitter and Instagram Accounts. 

On Wednesday August 7, 2019, the program’s new hours will be from 7:30 am—7:30 pm, Monday through Friday, with no weekend or holiday hours and the program will continue to be contacted as follows: 

  • Scheduling Line: 410-396-7633
  • Facility Line: 410-396-7434
  • Problem Line: 410-396-7635

Hospital Discharges

Starting August 5, 2019 All Community Partners must call Hart to Heart (443-573- 2073) directly  to schedule hospital discharges as follows

  • Monday through Friday between the hours of 7:30 pm to 7:30 am and;
  • Friday 7:30 pm to Monday 7:30 am

Senior Center Activities Suspended through January 31st
