A Letter of Non-Contagious Disease may be required by embassies for the purposes of transporting a deceased body to another country. Funeral homes can request this letter from the Office of Acute Communicable Diseases for Baltimore City residents.
Required Information for Submission:
- Name and Address of Funeral Home
- Name of Requestor
- Copy of Death Certificate
- Email address (or fax number)

Please note: The Office of Acute Communicable Diseases will only issue Letters of Non-Contagious Disease for deceased residents of Baltimore City. Requests for individuals who were not residents of Baltimore City will not be processed. Please contact the respective local health department where the decedent was a resident.
