2023 - 2024 Community Health Needs Assessment (CHNA)

The Community Health Needs Assessment (CHNA) is a comprehensive report that identifies key health challenges within the City of Baltimore. It is data collected in partnership with local hospitals to provide a clear understanding of the health priorities and needs within the community. The CHNA highlights access to care, mental health, social determinants to health, chronic disease, substance use disorder, health equity, and food resources.

The Community Health Needs Assessment aims to engage community members, healthcare providers, and local organizations in addressing these health challenges in a collaborative way. It serves as a foundation for developing strategies and programs that promote the community’s well-being, improve health outcomes, and reduce health inequities. Within the assessment, the identified priority areas help guide community health initiatives and resources toward areas that need the most attention.

This assessment is a part of the Baltimore City Health Department’s commitment to improving the health and well-being of all residents within the City of Baltimore and will inform future public health actions.

BCHD Community Health Assessment (CHA)

Our role as the Baltimore City Health Department (BCHD) is to understand the health of our communities. A Community Health Assessment (CHA) is conducted every three years by the Baltimore City Local Health Improvement Coalition (LHIC) to learn more about the factors that affect our well-being. The information in the CHA is a snapshot of Baltimore City’s health and well-being. 

The CHA summarizes community insights and population data. The dashboard can be used as a guide to prioritize health concerns, create solutions, and use resources effectively to address issues in the community.
